
This infographic summarizes a telephone adaptation of the Community Paramedicine at Clinic (CP@clinic) program implemented during the COVID-19 pandemic for older adults living in social housing. Community paramedics conducted telephone-based screenings for COVID-19 concerns, emergency preparedness, social isolation, and food insecurity while providing education on staying safe, self-isolating, and self-monitoring. The infographic presents participant characteristics, social health screening results, and key themes identified from paramedics’ notes, including daily life adaptations during the pandemic, emotional experiences such as loneliness and coping strategies, and the support provided by community paramedics. The findings highlight the value of CP@clinic as a way to maintain access to healthcare services, health information, and social support for older adults during a period of significant public health need.
